GOAL
Recognizing that homelessness and health are closely linked, our goal is to improve the overall wellbeing of homeless individuals through primary care and case management services offered on site and in collaboration with other service providers in the Durham area.
WHO WE SERVE
Lincoln’s Health Care for the Homeless Clinic welcomes men and women (18 years old and above), who are living in emergency shelters, transitional housing, domestic violence shelters, on the streets, in cars, in the woods, or doubled up in unstable housing. We will provide services for up to one year after the individual has obtained stable housing, and will assist with transition to another provider at that time if necessary.

Please note that we operate on an appointment schedule but will make every attempt to see Walk-in patients when possible. Walk-in patients are seen on a first-come, first-serve basis.
HOSPITAL FOLLOW-UPS

However, we are only available to see patients 20 hours per week, therefore scheduling can be limited. Please call as soon as you know a patient has a discharge need. Every attempt is made to see Hospital follow ups within 3- 5 days.
To schedule a follow-up appointment please call (919-808-5640). Leave a message if needed as the clinic is closed during the morning hours.
Please consider prescribing medications from our pharmacy formulary in order to ease the transition of care (Drug Formulary).
