Health Educator/Clinical Health Educator-LPN


OCCUPATIONAL SUMMARY

Provides individualized self-management education/training to individuals and groups that is recognized as evidenced-based and reflects the current standards of care.
Coordinates holistic continuity and follow-up care for patients to include Care Coordination services, Behavioral Health services, clinical care and community care networks.

Data collection, evaluation and reporting of health and education trends for assigned patient population of Lincoln Community Health Center.

Provides individualized self-management education/training to individuals and groups that is recognized as evidenced-based and reflects the current standards of care.

Safely performs routine skills as defined as by LCHC Department of Nursing and within the scope identified by the North Carolina Board of Nursing.

Observes and reports concise and accurate patient information to appropriate personnel within a time frame that allows effective action.

Participates in the evaluation of patient care outcomes.

QUALIFICATIONS

Graduation from an accredited College or University with a Degree in Health Education, Public Health or related field; Masters preferred. Must have CHES certification.
Completion of Licensed Practical Nurse curriculum from an accredited nursing program. Must have current nursing license to practice in the State of North Carolina.
