CASE MANAGER – MAI-SI Project
(Behavioral Health Clinic)
The case manager will provide comprehensive case management services for participants served by the MAI-SI project. This includes providing assessment of patient needs; coordinating all aspects of care, including behavioral health primary care health, HIV and hepatitis treatment, and other supportive services (e.g., housing, benefits, employment); researching resources; and referring, linking, and coordinating services to ensure client access to all available resources to meet identified needs.
KNOWLEDGE, SKILLS AND ABILITIES
Ability to communicate clearly in writing and verbally in a professional manner and style, and the ability to communicate effectively with people of diverse backgrounds and educational levels. Ability to maintain an effective working relationship with patients, staff, community partners and the general public. Knowledge of or ability to obtain knowledge of community resources. Strongly prefer ability to communicate in English and Spanish.
EDUCATION AND EXPERIENCE
Bachelor Degree in Human Service area such as social work or psychology. Two (2) years documented experience in the field of mental/behavioral health or case management in behavioral health, medical or social service agency. MSW preferred.
