Behavioral Health Professionals

(LCAS, LPC, SW or Case Manager)


Provides integrated substance abuse/mental health treatment to patients through case management, outreach, active coordination of care between community based providers (case managers), as well as the ID clinic providers. The BHP ensures timely and coordinated access to medically appropriate levels of health support services and continuity of care, through ongoing assessment of the client’s and other key family member’s needs and personal support systems. Actively supports the Lincoln Community Health Center’s mission of service, education, and research.


A general knowledge of Durham community and its resources is required. Interviewing, facilitation, and computer skills required. Cultural competence through ability to communicate effectively, verbally, and in writing to diverse populations including patients, staff, and general public required. Ability to perform with minimal supervision desired.

A MSW or Masters Degree in a related field. Licensed LCAS or LCSW preferred. Bilinigual (Spanish) preferred.. General knowledge or work experience in the health field.


One (1) year of working in a community/public health setting. General knowledge of the Durham community desired.
