BILINGUAL CARE COORDINATION REPRESENTATIVE

POSITION DESCRIPTION

The Care Coordination Representative works with Lincoln Community Health Center Referrals Department, Adult Medicine Clinic Advice Nurses and the Scheduling Department in order to ensure that patients receive appointments to specialized care providers in a timely manner; receive follow-up appointments with their primary care providers in a timely manner; and that there are available appointments for ‘hospital follow-up’ and ‘urgent care’ appointments for those patients in need of non-critical acute care.

KNOWLEDGE SKILLS and ABILITIES 

    • Strong communication skills and knowledge of telephone etiquette
    • High School diploma or equivalent
    • Computer skills (Microsoft Work, Excel, email, patient electronic records and internet)
    • Filing
    • Flexibility
    • Motivated to learn
    • Ability to multi-task as needed
    • Team worker
    • Bilingual (English/Spanish)
