Health Educator has primary responsibility for the provision of diabetes education including self-management goal setting, tracking diabetic patients and other chronic health conditions. The educator will administer the educational aspects of the Center’s primary health program and will utilize relevant resources including Core Curriculum for Diabetes Education published by the American Association of Diabetics Educators for patient education. Patient contact will involve one or more visits, phone tracing and group visits. The educator will be a member of a multidisciplinary team providing services to the clinic population.
Thorough knowledge of the Center’s operation and procedures; ability to skillfully and accurately provide self-management education to diabetic and other chronic disease patients; thorough understanding of the patient’s needs and how these can best be met by appropriate use of facilities, personnel and general knowledge of chronic health conditions, and personnel. Ability to communicate with skill, clarity and accuracy with the patients, physicians, and ancillary health care workers at all levels. Ability to learn basic computer skills for data entry and tracking. Bilingual Preferred.
Bachelors Degree in Public Health, Health Education or related field required. Master’s Degree Preferred.
