BEHAVIORAL HEALTH PROVIDER
(Early Intervention Clinic)
Provides integrated substance abuse/mental health treatment to HIV infected patients through case management, outreach, active coordination of care between community based providers (case managers) as well as the ID clinic providers. The BHP ensures timely and coordinated access to medically appropriate levels of health and support services and continuity of care, through ongoing assessment of the client’s and other key family member’s needs and personal support systems. Actively supports the Lincoln Community Health Center’s mission of service, education, and research.
A general knowledge of Durham community and its resources is required. Knowledge of HIV/AIDS through seminar and coursework required. Interviewing, facilitation, and computer skills required. Cultural competence through ability to communicate effectively, verbally, and in writing to diverse populations including patients, staff, and general public required. Ability to perform with minimal supervision desired.
MSW or masters degree in a related field. Licensed LCSW and LCAS. Some knowledge or previous work experience in the health field especially related to HIV/AIDS care.
One (1) year of working in a community/public health setting. General knowledge of the Durham community desired
